1. Who we are
Lifelink Support Services provides remote wellness, daily living and NDIS-aligned support across Australia. We comply with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth) and applicable NDIS Practice Standards.
2. Information we collect
- Identity & contact details (name, date of birth, address, phone, email)
- NDIS plan information, plan management type and support coordinator details
- Health information: allergies, medical conditions, mobility, fluid restrictions, medication schedules
- Wellness tracking data: blood pressure, blood glucose, weight, fluid intake, check-in notes
- Communication preferences (phone, SMS, WhatsApp, email, video)
- Family and emergency contacts you nominate
- Records of interactions with our staff and progress notes
3. How we use your information
We use your information solely to deliver and improve your support, including coordinating reminders, scheduling appointments, sharing updates with nominated contacts, and meeting our regulatory obligations.
4. Secure data handling
- Data is encrypted in transit (TLS) and at rest
- Role-based access controls — only assigned staff can view your record
- Audit logs record who accessed or changed information
- Multi-factor authentication for staff accounts
- Regular backups and least-privilege database policies
5. Role-based access
Administrators, assigned support workers and nominated family contacts see only the information necessary for their role. Inactive staff lose access immediately.
6. Wellness & medication disclosures
Wellness check-ins and medication reminders are supportive prompts, not clinical monitoring. They do not replace medical advice, GP supervision or emergency services.
7. WhatsApp & messaging
Where you have consented, we may communicate via WhatsApp or SMS. Standard provider terms apply. You may withdraw this consent at any time by contacting us.
8. Sharing your information
We share information only with: assigned Lifelink staff, contacts you have authorised, and regulators or emergency services where required by law.
