Information Collection Notice

This notice explains, in plain language, the personal and health information Lifelink collects during onboarding and ongoing support.

What we collect

  • Identity: name, preferred name, date of birth, gender, address
  • Contact: phone, mobile, email, postal address
  • NDIS: participant number, plan dates, plan management, support coordinator
  • Health profile: allergies, medical conditions, mobility, falls risk, communication needs, fluid restrictions
  • Wellness data: blood pressure, blood glucose, weight, fluid intake, medication confirmations
  • Family/emergency contacts: name, relationship, mobile, WhatsApp consent
  • Preferences: phone, SMS, WhatsApp, email, video
  • Documents: plans, consent forms and other records you upload

Why we collect it

To provide safe, personalised remote support; to coordinate reminders and check-ins; to share updates with people you nominate; and to comply with NDIS and Australian regulatory obligations.

How we collect it

Directly from you, your nominated representative, your referrer, or — with your consent — from your support coordinator or treating practitioners.

If we don't collect it

You can decline to provide information, but some services (e.g. medication reminders, family check-ins) may not be possible without it.

Storage & retention

Information is stored on secure Australian-region cloud infrastructure. Records are kept for as long as required by law and NDIS quality standards, then securely destroyed.
